Meet Our Team

Sarah Pack
is a Board-Certified Nurse Practitioner specializing in bariatrics who has accreditation in Obesity Medicine for Primary Care Providers with the American Association of Nurse Practitioners. Sarah has provided diagnosis and treatment of obesity and its related co-morbidities for the past three years and during that time, has been active in working with researchers studying the causes behind obesity. She has a particular interest in the link between genetics and psychology that factor in a person’s weight.

Jackie Cooke
is a member of the American Association of Nutritional Consultants. With a B.A. in communications, she has a proven track record of exceptional customer service and a heart for helping others achieve their weight-loss goals. She believes in this plan because it has helped her lose fifty pounds and eliminate her diagnosis of pre-diabetes. Jackie’s personal experience of weight-loss brings further understanding and compassion when she is seeing her patients.

We start with a free consultation that assesses patient’s health, goals, and needs that determines a customized roadmap to weight loss success. If you agree with that plan, we schedule your first visit for Active Phase 1, where you will have labs drawn, get an EKG, meet with our medical provider, and will be introduced to your personalized eating plan by our nutritional consultant.

We consider all the factors that are affecting weight including physical and mental aspects. We can also include medically backed tools that can help assist weight loss such as prescription appetite suppressants: phentermine, phendimetrazine, Qsymia, Contrave, Saxenda, Wegovy, and others are available for those patients who qualify. Vitamin injections are another tool available.

During your second visit, you will begin Active Phase 2, where we incorporate a specialized exercise plan that is tailored to your physical abilities. After that, you will meet with our medical and nutritional staff weekly for accountability and emotional support to make sure you stay on track. When you have met your weight loss goals, we transition you out of the Active Phase 2 into our maintenance program.

The maintenance program provides guidance on keeping the lost weight off. We provide guidance for a healthy way of life for you that is sustainable.
